The rule: 
a form of TO BE + Voltooid deelwoord

We use a passive sentence to emphasize on the action instead of the person/thing doing it. And we always use the same tense!

How to make an active sentence passive?

Step 1: find subject, verb and object.
Step 2: Make object new subject.
Step 3: add a form of ''to be'' after the new subject.
Step 4: finish the sentence. 

The boys ate the pie.
The pie was eaten (by the boys).
