How do we form a passive sentence?

1: Scrap the subject from our sentence (create an empty space)
2: find the Direct or Indirect Object in our sentence
3:  make object (DO/IO) new 'subject' 
Why use passive?

To focus is on what is happening, not on who does it!! 
You report on something

Passive: how?

Use a form of: to be + past participle
Examples: 

present tense
A: He walks the dog every evening.
P: The dog is walked every evening

past tense
A: Yesterday,  my phone broke.
P: My phone was broken

'by zombies/grandma'
OR by + original subject 

A: I explain grammar every lesson.
