Affirmative (+) sentences: had + past participle (3rd form)
I had eaten lunch...

Negative (-) sentences: had + not + past participle (3rd form)
I had not eaten lunch... or I hadn't eaten lunch

Questions (?): had + subject + past participle (3rd form)
Had I eaten lunch...?
