I know that Revere is riding out of the city, and these lines tell me he is warning people living in the country.
Based on the facts, Middlesex must be a place outside of Boston.
I have never heard of a colony called Middlesex so we can INFER that is a county including Boston.

He is spreading the alarm to "UP AND TO ARM"
This means that the villagers need to prepare to fight.
