Use: for facts / general truths
Form: if + present simple, present simple

First conditional
Use: refers to a possible condition and its probable result.
Form: if + present simple, simple future

Second conditional
Use: refers to an unlikely or hypothetical condition and its probable result.
Form: if + past simple, would + infinitive

3rd conditional
Use: It talks about the past. It's used to describe a situation that didn't happen, and to imagine the result of this situation.

Form: if + past perfect, ...would + have + past participle

Example: If she had studied, she would have passed the exam. 
(but, really we know she didn't study and so she didn't pass)
