Would you like to practise with using the two tenses (not having to choose), then use the one with the checks/question marks/crosses. Do you want to practice chosing the correct tense in a sentence, then choose the one with gap sentences. You will need an online dice for both. Write down your answers on the piece of paper you will be given.
