How to use the passive voice?

Rembrandt had painted De Nachtwacht.                    De Nachtwacht had been painted by Rembr.

Trump has fired Mark Esper.                                               Mark Esper has been fired by Trump.

I saw an accident yesterday. Two people..... (take) to hospital.

He must fill in the form.

She is carrying all the boxes.

The motorway... (close already) for three days.

Which three steps do you have to take to turn a sentence into a passive?
